Contacting the office
The contact form asks for your name, email address, phone number, insurer name and an optional general question. The website sends this information by email to the configured office recipient so the office can respond. An email address you enter is used as the reply address.
Use this form for general inquiries only. Do not include medical records, symptoms, diagnoses, medication details, insurance identification numbers or other sensitive information. Call the office for instructions about patient matters. Sending a form does not confirm an appointment. For a medical emergency, call 911.
Website statistics and technical information
The hosting service records website requests. These server logs can include IP addresses, requested pages, request times, referring addresses and browser information. AWStats uses hosting data to produce traffic statistics. Weekly email summaries contain aggregate counts and selected public page totals; they do not include visitor IP addresses or form contents.
The website uses a temporary session cookie to validate contact-form submissions and limited technical records to help restrict repeated submissions. The contact-form application does not keep a separate message database; messages are sent through the hosting mail service and can be retained in email systems. Hosting and email providers also process technical information to operate their services.
