SPECIALIZED ENDOCRINOLOGY CARE

Basic membership

$1,200 / year

For patients who keep their own primary care physician and see Dr. Kargutkar for endocrinology care.

  • Four quarterly follow-up visits per year
  • Priority appointment scheduling
  • Direct physician cell phone access for emergencies

Additional appointments are charged separately based on the time and complexity of the visit.

PRIMARY CARE & WELLNESS

Full membership

$2,500 / year

Establish Dr. Kargutkar as your primary care physician, with additional wellness services included.

  • Annual physical examination
  • Four quarterly in-person follow-up visits
  • Additional in-person or telemedicine visits as needed
  • Priority scheduling and emergency physician cell phone access
  • Six 30-minute Emsella sessions
  • Six 30-minute EMSlim body sculpting sessions
  • 20% off supplements and aesthetic treatments

In-network plans

  • Blue Cross Blue Shield under the InfiniHealth IPA

Horizon Omnia plans are subject to Tier 2 rules and deductibles. Confirm your specific plan and benefits with the office and your insurer.

Out-of-network care

Cigna, United, AmeriHealth, Qualcare, and Oxford are listed as out of network. Confirm your specific coverage with the office and your insurer.

Itemized superbills are provided after each visit for you to submit for potential reimbursement. Ask your insurer to send reimbursement checks directly to you.

How can I pay?

HSA and FSA cards are accepted. Payment options include cash, debit, credit cards, Zelle (AceEndoOffice@gmail.com), and Venmo (@aceendo). Credit cards carry a 3.5% processing fee. Personal checks are not accepted.

Which membership fits my needs?

Basic membership is for endocrinology care while you retain your own primary care physician. Full membership establishes Dr. Kargutkar as your primary care physician and includes the additional services listed above. Call the office to discuss your care needs and enrollment.
