Welcome to our Concierge Medicine practice with Dr. Smita Kargutkar.
We offer structured membership options designed to deliver personalized, accessible, and comprehensive healthcare tailored to your needs.
Our Full Membership ($2,500 per year) establishes Dr. Smita Kargutkar as your Primary Care Physician. This comprehensive plan includes an annual physical examination, four quarterly in-person follow-up visits, and additional in-person or telemedicine appointments as needed. Members enjoy priority scheduling and direct access to the physician's cell phone for emergencies. This tier also includes advanced wellness and body contouring services: six 30-minute sessions on the Emsella Chair for pelvic health, six 30-minute sessions of EMSlim Body Sculpting, and 20% off all supplements and aesthetic treatments.
Our Basic Membership ($1,200 per year) is designed for patients who retain their own Primary Care Physician and see Dr. Kargutkar solely for specialized endocrinology care. This plan includes four quarterly follow-up visits per year, priority appointment scheduling, and direct emergency cell phone access to the physician. Any additional appointments will be charged separately depending on the time and complexity of the visit.
Insurance & Payment InformationWe are in-network with Aetna and Meritain Aetna, as well as Blue Cross Blue Shield under the InfiniHealth IPA (Tier 2 rules and deductibles apply for Horizon Omnia plans).
While we are currently out-of-network with Cigna, United, AmeriHealth, Qualcare, and Oxford, we will update our website immediately if our network status changes.
We accept HSA and FSA cards.
For out-of-network care, we provide itemized superbills after each visit for you to submit to your insurer for potential reimbursement; please instruct your insurance carrier to issue reimbursement checks directly to you rather than the practice.
