Aetna Commercial Patients:

• We will be in-network with Aetna Commercial Insurance in 2025-2026 and existing Aetna Commercial patients can continue seeing the physician as usual. 
• We are accepting New Aetna or Meritain Aetna patients.
• We do not accept any other insurance.
• SELF PAY: $450 for Initial visit and $250 for follow-up appointments.
• We accept HSA/FSA cards and will give you a superbill after every physician visit, which can be submitted to your insurance company for possible reimbursement. Ensure you contact your insurance provider and instruct them to forward the payment to you, not the practice.
• MEMBERSHIP OPTIONAL: The membership fee is $1200-2500 per year; see below to see what the membership covers

• Full membership: $2500 PER YEAR Comprehensive annual physicals every year, 4 in-person follow-up visits every 3 months, 6-30 minute sessions on Emsella Chair to help with urinary incontinence for women and erectile dysfunction for men, EMSlim Body Sculpting: 6-30 minute sessions for EMSlim body sculpting, to build muscle and burn fat, 20% off supplements and aesthetic treatments. Priority appointments and access to the physician for emergencies.
• Basic Membership: $1200 per year covers 4 follow up visits in the year every 3 months and priority appointments and access to the physician for emergencies.
