If you would like to leave the practice, please complete the record release form on the website (click on information—Record release form) and email or fax the completed signed form with your new physician's information and phone and fax number. 

We will refill your medicines for a maximum of 6 months from the last visit as a courtesy. Weight loss and high-risk medicines can only be refilled for 3 months after the last appointment. We will NOT refill any medicines after this period.
